How do I obtain a copy of my child’s visit summary?

To download a PDF of your child’s visit, please follow the steps below:

1. Log into your account through the Blueberry app or our website.
2. Tap on “Children” from the bottom menu.
3. Select your child's profile.
4. Under “Completed Visits,” choose the visit you need.
5. Tap “Download Report as PDF” below your child’s name and diagnosis at the top of the screen.
