For Blueberry to release medical records, the account holder must grant access. Written consent is required for account access. We can assist by reaching out to the other parent on your behalf to request medical records. If the account holder provides authorization, we will file this and release the records as needed.
Please contact us at help@blueberrypediatrics.com and include the following details:
- Your child’s full name and date of birth.
- Your preferred contact information (email address or phone number).
- Details of the specific records you are requesting (if not requesting the entire record).
Please note: Unlike traditional doctor’s offices, Blueberry cannot release medical records based solely on custody documentation. Since our platform operates through account holders, written consent from the account holder is required to access and release any records.
