- Child Name:
- Child DOB:
- Are you registering via your child’s insurance, your employer, or a Blueberry partner?
If registering through insurance, also include the following
- Insurance Name
- Child's Member ID/Medicaid ID
- A copy of your child's insurance card
If through an employer or Blueberry partner, also include the following:
- The name of the employer or partner.
- Any additional email address that may have been used to create your file
If you need to start a visit for your child today, please let us know in your email. We will do our best to prioritize and escalate your request.
Please note that these steps are for children who have been deemed eligible by their insurance provider, employer, or Blueberry partner for a Blueberry membership. If you’re having trouble adding your child through our app for a paid membership, please try adding your child by logging into your Blueberry account via the web at www.blueberrypediatrics.com using your phone or computer browser.
