If you are eligible for a Blueberry membership through Community Health Choice, please follow the steps below to create your account:
Step 1: Access the Sign-Up Page
Click here to visit the sign-up page, and then select the blue “Claim your benefit” button as shown below.
Step 2: Create Account
When prompted, enter your first and last name, email address, and the password you'd like to use, and agree to receive SMS messages from our service, then click "Continue."
Step 3: Verify Eligibility
Enter your child’s first and last name and date of birth, OR your child's insurance member ID, OR your child's Medicaid ID to confirm eligibility when prompted, then click "Check eligibility."
Step 4: Claim your kit
Once eligibility is confirmed, enter your phone number and shipping address in the designated fields and click "Join."
Step 5: Add Additional Children
If eligibility is confirmed, you’ll have the option to add any other eligible children. We recommend adding all eligible children during this step to ensure a smooth registration process.
